Final Document Submission Form

Once an IIR study has concluded, an Investigator must complete and submit a Final Document Submission (FDS) via TAP website iir-tap.com.

According to the terms of the Study Agreement, the Investigator must submit a copy of an abstract or presentation and a manuscript, resulting from the study to TAP for courtesy review at least 30 days prior to submission to a conference or journal according to the terms of the Study Agreement. A final report may also be requested.

The Investigator will receive notification of TAP's written comments from the IIR staff.

Resource Request Accountability

Funding – Upon completion of the last milestone set forth in the Study Agreement (e.g., presentation or abstract and manuscript), the Investigator must submit a final payment requested via TAP website iir-tap.com.

Clinical Supplies – Upon completion of the last milestone set forth in the Study Agreement (e.g., presentation or abstract and a manuscript), the Investigator must document the return and/or destruction of any remaining supplies by e-mail to iir@tap.com TAP.
